Department Overview

The Kingman Fire Department is a municipal fire department that serves and area that encompasses 34.82 square miles. The services are provided throughout the city limits as well as mutual aid and automatic aid with neighboring jurisdictions.  The department consists of career employees providing prevention and response activities in a comprehensive, all-hazards response model including fire suppression, advanced life support in emergency medical services, hazardous material, technical rescue, wildland, fire investigation, domestic preparedness planning and response, as well as public education and prevention.
The current personnel roster consists of 91 fulltime career personnel, 15 part-time personnel, 3 civilian support personnel, 1 building official, 3 building inspectors, and 3 hydrant maintenance personnel.  In addition, the Fire Department manages the 911 communications center which consists of 1 Communications Administrator, 4 crew leaders, and 11 communications specialists.  In the operations division, 43 career employees are on three rotating shifts of 14 personnel each working 24 hour shifts, providing both emergency and non-emergency response.  Each shift is managed by a Battalion Chief.  
Administration, Dispatch, Building and Life Safety, Training and Education are the other divisions necessary for our well-rounded department, each of which is supervised by a Battalion Chief level supervisor. The Kingman 911 Center is a stand-alone center that employs ten full time communications specialists, four communications specialist crew leaders, and one communications administrator.  All communications specialists are certified Emergency Medical Dispatchers.

Obtaining Fire and EMS Reports

To obtain a copy of a fire report or medical report for a patient who was not transported to a hospital, please complete the Request Form(click here) and return to:

You can also  contact the fire department at 412 East Oak Street, Kingman, AZ 86401 or (928) 753-2891. It is reasonable and necessary to charge a nominal per page fee to offset the cost of paper, supplies and equipment. The City charges $0.20 per page after the first two pages which are free. Proper identification will be required to obtain these records as required by the HIPAA (Health Portability and Accountability Act of 1996) policy.

Medical reports for patients who are not transported to the hospital can only be obtained:

If patient is 18 years of age or older with one of the following:

  • Requestor is the patient and has a photo ID.
  • Requestor has a Kingman Fire Department authorization of Disclosure of Protected Health Information form (click here) and a photo ID.
  • Requestor has a valid power of attorney or court order for the patient and photo ID.

If the patient is under 18 years of age, one of the following is required:

  • Requestor has an original or certified copy of the patient’s birth certificate.
  • Requestor has an original or notarized copy showing Court appointed guardianship of the patient.
  • Requestor has an original or certified copy of the patient’s birth certificate or Court appointed guardianship papers and a notarized letter stating that the parents or guardian allow the requestor to have the information.

In the event the patient was transported to the hospital, our report becomes a part of the hospital's patient file. Copies are easiest to obtain through Kingman Regional Medical Center where a patient can request a complete copy of their file.
